PIANO & VOICE REGISTRATION

Registration for piano and voice lessons is now open for the 2024/2025 school year.
Students in 1st through 12th grade may enroll in piano lessons.
Students must be at least twelve years old or receive approval from instructor to enroll in voice lessons.
If you would like to register your child, please read the information below, complete the registration form, and submit it, along with a $15 registration fee.
The Avery County school year calendar will be "loosely" followed, which will consist of 36 private lessons, 30 min in length.
Classes meet once a week on Thursdays, beginning on September 1st, 2022.
The season will conclude with a spring recital at the end of May or early June.
If a lesson is missed due to sickness or the teacher’s absence, a strong attempt will be made to provide a make-up lesson.
The cost is $80/month or $720/year for 36 private lessons.
Monthly payments must be received by the 7th of the month, whether the student attends lessons or not. We have a specific lesson time reserved for each student, and the teacher is paid for this time regardless of attendance.
Monthly tuition will be calculated based on start date.
The cost of lesson books is included in monthly tuition.

Registration is not complete until registration fee is processed and received.

Liability Release and Waiver

Release of Liability
As the legal parent or guardian of the student named above, I understand that my child's participation in music lessons with Blue Ridge Performing Arts Academy (BRPAA), whether on or off premises, may expose him/her to unforeseen risk of personal injury, death, or property damage. I hereby acknowledge that I am voluntarily allowing my child to participate in all lessons and events and agree to assume any such risks.

I hereby release and hold harmless BRPAA, its owners, instructors, students, employees, and owners and lessors of premises from any claims, demands, costs, expenses, or liability incurred as a result of injury or mishap sustained in association with participation in music lessons. Additionally, I release and hold harmless BRPAA from any liability should I, the student, or other associated third parties sustain any personal injury caused by the ordinary negligence of BRPAA, its owners, instructors, students, employees, or owners or lessors of premises.

Medical Release
I give my consent for my child to receive emergency medical care and/or be transported by ambulance or other conveyance to a doctor or hospital for attention or treatment in the event of a medical emergency. I understand that any medical expenses will be billed directly to me or my insurance company.

Photo/Media Release
I agree to permit Blue Ridge Performing Arts Academy staff, agents, and other guests to take photographs and/or video and audio recordings during studio lessons and performance events without further recourse. I understand that such photographic images, video, or audio recordings of my child may be used for editorial, fundraising, and/or promotional purposes, and in any manner and medium including, but not limited to, web pages, Facebook, Instagram, flyers, posters, etc. I further understand that I may opt-out of this release with respect to photographs, videos, and audio only by completing and signing a Media Opt-Out form available at the BRPAA front desk.

A Covid Waiver will be sent to you via email.

I, on behalf of my minor child, hereby warrant that I have read this Liability Release and Waiver Form in its entirety and fully understand and accept its contents. By typing my name below, I hereby give my minor child permission to participate in music lessons with Blue Ridge Performing Arts Academy. I understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature.
